Real Choices Medical Plans
Assurant Health’s small group health insurance plans, aptly called Real Choices, include the most popular options in health care coverage today. The flexibility of these plans enables small group employers to achieve a balance between the needs of employees and the bottom line.
• Real Choices I
Real Choices I offers employers the most extensive coverage. With nine deductible choices that range from $0 to $10,000, the plan is sure to please the established business owner. Additional benefit highlights include:
- $5 million lifetime benefit maximum included. Option to increase to $10 million
- $750 first-dollar preventive care benefit included. Option to increase to $1,250
- Optional $0 copay for generic prescription drugs. Prescription drug benefits are managed by Medco
- Four choices of office visit copays for primary care physician, ranging from $20 to $50
- Maternity coverage that give employer a wide choice of benefit options
• Real Choices II
Real Choices II is extremely popular because it offers the most choices. Nine deductible choices and five benefit percentage choices make this plan one that can be designed to fit the employees’ unique needs. Other benefit highlights include:
- $2 million lifetime benefit maximum included. Option to increase to $5 million
- $300 first-dollar preventive care benefit included. Option to increase to $800
- $200 first-dollar diagnostic x-ray and lab services benefit included
- Five optional prescription drug benefit choices, including a $0 generic drug copay

• Real Choices III
Real Choices III is designed for the employer with a new business or one that has no health insurance. Real Choices III is our most affordable plan and is available without having quality coverage compromised. Benefit highlights include:
- Optional $40 per office visit first-dollar benefit
- $1 million lifetime benefit maximum included
- $200 preventive care benefit included
- Optional first-dollar accident medical expense benefit of $500 or $1,000

Assurant Health has been an industry leader in designing consumer-driven health plans since they began.
Assurant Health has designed two plan choices for HRAs and two plan choices for HSAs. These tax favored plans open a wide range of choices for both the employer and the employees. Benefit choices include:
- Wide range of deductible and benefit percentage choices
- First-dollar preventive care benefit included
- Optional first-dollar accident medical expense benefit
- Choice of lifetime maximum benefit from $2 million to $10 million, depending on plan selection
Real Choices Ancillary Plans
Real Choices ancillary coverage includes life insurance, short-term disability insurance and dental insurance. These attractive benefits can be added to your medical plan and a premium discount is given for each ancillary product selected—up to 15% off the ancillary premium!
Life Insurance
Up to $30,000 per employee guaranteed issue (amount depends on medical plan selected). Additional amounts can be purchased up to $500,000.
Short Term Disability
You can select coverage up to 66% of an employee’s weekly salary. Two plans are available—amount is based on a percentage of employee’s salary or a flat amount from $50 to $1,000 per week.
Dental Insurance
Select a plan that best suits your needs! Real Choices dental insurance has six plans available. You can enjoy first-dollar coverage for preventive services with no deductible or coinsurance to satisfy! Dental insurance can also be purchased as a stand-alone product for groups of three or more.
Tax Saving Arrangements
Premium Only Plans
Assurant Health’s Premium Only Plan (POP) is a simple and convenient way to cut taxes for employees and employers. Made possible through Section 125 of the Internal Revenue Code, a POP can increase employee take-home pay and reduce a company’s payroll taxes. But in addition to the tax savings, another exciting point about an Assurant Health POP is that it’s FREE!
